You can breed your horse to an unrelated horse to limit the inbreeding but it only helps to a certain extent so there is not really a way to fix it and even if there is a way it is nearly impossible. It's worth noting that nothing in the game looks at the COI% to do any calculations. If the horse has good genes the game doesn't care where they came from.

Not directly, but a bit more indirectly. We were told that higher COI will eventually mean weaker horses in some aspects. When such a thing comes, we are usually given a LOT of warning, so we can prepare. So, keep it in mind, get used to working with COI but overall, don't concern yourself too much about it.
