COI represents the level of inbreeding in a horse, the higher the COI the more the horse has been inbred. As to impact, a horse that has been highly inbred will be more likely to pass on its genes both good and bad.

As regards what to look for in a stud it very much depends on what you want out of the breeding. If you're looking to breed a horse for competitions then you would look for a stud with a proven good competition history and also greens or gold in the stats that are applicable to the competitions you want to enter.
